Once your appointment is scheduled, you will receive a packet of paperwork in the mail. Please complete the enclosed forms and bring them with you to your appointment. Alternatively, you may print out these forms from the link at the bottom of the page labeled Common Forms.
What to Bring with You
Please arrive approximately 15 minutes prior to your appointment so we can ensure that all the appropriate information is ready for your physician and health insurance plan.
When you come to your appointment, please bring your insurance card, photo ID, and any required co-payment to every appointment along with a referral from your physician, if necessary for your insurance. Confirming that we receive the necessary paperwork required by your insurance plan is your responsibility.
In addition, please bring any additional medical information (including films, lab work etc) that you were asked to bring when you scheduled your appointment. If you were not asked to bring anything, that is ok. Not all patients are required to bring medical information with them.

Office Appointment Policy
Our office appointments include patients scheduled to see surgeons and/or Nurse Practitioners in consultation for surgery, as follow-up to surgery and/or for diagnostic studies in our Vascular Laboratory. Because of our various services, appointment lengths may vary. Patients are not necessarily seen in order of arrival.
We apologize if you have to wait to be seen. However, cardiac, vascular and thoracic surgical care can be urgent in nature, and it is our practice to treat those patients in urgent need immediately.
We appreciate your understanding and cooperation.
