Office Appointments

CVTSA office hours are from 8:00 a.m. to 4:30 p.m. Monday through Friday.

Once your appointment is scheduled, you will receive a packet of paperwork in the mail. Please complete the enclosed forms and bring them with you to your appointment. Alternatively, you may print out these forms from the link on the left labeled Common Forms.

 

What to Bring with You

Please arrive approximately 15 minutes prior to your appointment so we can ensure that all the appropriate information is ready for your physician and health insurance plan.

 

When you come to your appointment, please bring your insurance card and any required co-payment to every appointment along with a referral from your physician, if necessary for your insurance. Confirming that we receive the necessary paperwork required by your insurance plan is your responsibility.

 

In addition, please bring any additional medical information (including films, lab work etc) that you were asked to bring when you scheduled your appointment. If you were not asked to bring anything, that is ok. Not all patients are required to bring medical information with them.

Office Appointment Policy

Our office appointments include patients scheduled to see surgeons and/or Nurse Practitioners in consultation for surgery, as follow-up to surgery and/or for diagnostic studies in our Vascular Laboratory. Because of our various services, appointment lengths may vary. Patients are not necessarily seen in order of arrival.

 

We apologize if have to wait to be seen. However, cardiac, vascular and thoracic surgical care can be urgent in nature, and it is our practice to treat those patients immediately.

 

We appreciate your understanding and cooperation.

 

Emergency Situations

We also have a physician on call 24 hours a day, 365 days a year. After regular office hours, you may contact our answering service by calling your physician's office phone number.
