For your convenience, you can print out the forms here, after which you will fill them out and bring with you to your appointment.

  3. Authorization for Release of Information (HIPAA) Form
  4. Authorization for Release of Medical Records
  5. CVTSA Summary of Notice of Privacy Practices 

 

Your paperwork needs to be updated annually. If your personal information changes in the interim, including insurance information, change of address, etc. - please let us know so that we may better serve you.
